You must fill out a refrigeration agreement declaration if you are a licensed funeral provider who:
uses refrigeration services from another licensed funeral provider, of a different legal entity, or
provides refrigeration services to another licensed funeral provider of a different legal entity.
If you obtain refrigeration from anywhere that is not the address on your licence you must complete a refrigeration agreement. (If you have your own approved refrigeration facilities and you don’t rent or provide spaces, you don’t need to send in a refrigeration agreement.)
Getting or renewing your licence with us is contingent on having an up-to-date and approved refrigeration agreement.
As a funeral provider, you must prove that you have enough refrigeration to accommodate the reported number of human remains. If you don’t have your own on-site refrigeration storage, you can rent or obtain spaces from another licensed funeral provider. A refrigeration agreement declaration is a form that tells us about the arrangement between licensed funeral providers around the provision/receipt of refrigeration services.
Both parties must sign the agreement – the licensed funeral provider who uses/rents refrigerated storage space from another licensed funeral provider and the licensed funeral provider who provides/rents acceptable refrigerated storage spaces to another licensed funeral provider.
We only require one copy of the agreement and either party can send it to us.
We will cross-reference all declarations to make sure that there are enough spaces to accommodate all the businesses involved.
You must inform us of any changes before or as soon as they happen. You cannot operate without approved refrigeration. To notify us of changes to your refrigeration arrangements, please complete a new refrigeration agreement form with your new provider. We will then review and approve the changes before you can continue to operate.
Refrigeration providers must also inform us if you are no longer supplying storage to another licensed business.
